Who it is for

From hospital back to your home.

After an operation, an accident or an acute illness, you may leave hospital – but you are not quite back on your feet yet. Acute and transitional care exists for exactly this period.

  • After discharge from hospital

When you still need care after your hospital stay to become independent again at home.

  • With a clear goal

The care aims to get you back to the condition you were in before your hospital stay.

  • For relatives and professionals

Relatives, hospitals and social services can contact us directly so that care can be planned in good time.

What it includes

Care for the first two weeks.

Acute and transitional care covers the nursing services ordered in hospital for the period after your discharge – from basic care to treatment care.

  • Preparing and giving medication
  • Checking blood pressure, pulse and other readings
  • Help with personal care and with dressing and undressing
  • Mobilisation and exercises
  • Advice on what happens after transitional care

We obtain aids and care supplies for you – as agreed with you.

Costs & health insurance

Who pays for transitional care?

Acute and transitional care is paid for under the rules of hospital financing: your health insurer and your canton of residence share the costs, with the canton paying at least 55 per cent.

As with other services covered by basic insurance, you pay your deductible (franchise) and co-payment (Selbstbehalt). The patient contribution that applies to other nursing care is not charged for acute and transitional care.

Financing

Hospital financing

  • No patient contribution. It does not apply to acute and transitional care.
  • Deductible and co-payment as with other services covered by basic insurance.
  • Up to 14 days. After that, care can continue as regular Spitex care.
